Cloud County Health Center Releases Community Health Needs Assessment Survey

Cloud County Health Center and other community stakeholders are asking for community assistance in identifying health needs. Every three years, CCHC is tasked with identifying the most pressing health needs of the community and developing a plan of action to address those needs. CCHC works with a committee of community stakeholders to develop and distribute the survey to the community. The survey will be open until June 15th. After results are evaluated, the needs will be ranked and addressed with an action plan.
