FREE Sports Physicals

Sports physicals, also called a pre-participation exam, are needed before athletes can begin competing. The exam will help determine that the athlete is healthy enough to participate and ensure that there are no underlying health issues.

Playing a sport is a great way for kids to get active and stay in shape.

Please bring signed & completed Pre-Participation Physical Evaluation forms to your appointment. Copies may be picked up to complete prior to your appointment at the Family Care Center or can be downloaded/printed by going to: HERE.
