Care Management

The Care Manager is here to assist patients, their families and the community by providing:

• Information and referral for home care services such as home health agencies, hospice, and delivered meal services
• Medical equipment and oxygen
• Information and arranging assisted living, skilled care and nursing home care
• Information about possible financial resources
• Information about and assistance in completing Advance Directives, which includes Living Wills and Durable Power of Attorney for Health Care Decisions
• Emotional support during a difficult medical crisis
